How do we know if a death was preventable?
Data tells us the causes of deaths, but life and death are complex. Information from inquests and coroner reports provide rich insights into preventable deaths.
Regulations are in place in England and Wales to share concerns and prompt action to prevent future deaths. These regulations require coroners to write a report following an inquest if the death could have been prevented.
These reports are called Prevention of Future Deaths reports or PFDs.
